New York State Route 15A is a north-south running highway in western New York. Route 15A begins and ends at NY 15 at intersections 35 miles apart. The southern end of NY 15A is in the Livingston County town of Springwater. Its northern terminus is in the Monroe County city of Rochester.

Heading north from Springwater, Route 15A, named Bald Hill Road, enters Ontario County. The route then reenters Livingston County near Hemlock. In the town center, Route 15A becomes South Main Street and, just after changing names, intersects U.S. Route 20A. US 20A west then forms a duplex with Route 15A along the length of North Main Street. After two miles, US 20A turns east towards Buffalo while NY 15A continues north and turns into Plank Road.
Shortly before entering Lima, NY 15A becomes Lake Avenue. At the village center, Route 15A crosses over NY 5 and US 20, becoming Rochester Street in the process and retaining this name until the Livingston-Monroe county line near Honeoye Falls, where Route 15A becomes Rush-Lima Road.

Just northeast of Honeoye Falls in Rush, NY 15A begins curving to the east. At an intersection with Rush-West Rush Road, NY 15A turns back to the north and becomes East Henrietta Road, the name it retains until the end of Route 15A in Rochester. North of this intersection, after crossing the Genesee River, NY 15A intersects NY 251. Farther north, in the town of Henrietta, Route 15A passes over Interstate 90 and intersects routes 253 and 252, as well as interchange with Interstate 390. NY 15A meets up with I-390 once again in Brighton before passing over the Erie Canal and reaching its northern terminus at Route 15 in Rochester.

The NY 15A designation is actually older than NY 15, as NY 15 was US 15 until 1974, long after NY 15A was commissioned.
